A Case of Cervical Fracture-Dislocation without Neurological Deficit

Abstract
A case of severe fracture-dislocation of C4 on C5 which revealed no neurological deficit is presented with the review of the relevant literatures.
The patient was a 39-year-old Korean woman who complained of nuchal and suboccipital pain and limitation of the range of motion of the neck after sustaining cervical hyperflexion injury about 3 months before admission. Neurological examination revealed no deficit except kyphosis on midcervical region. Cervical spine demonstrated marked anterior dislocation of C4 on C5 with angulation and chip fracture of C5 body.
Skeletal traction was performed using Crutchfield tong with the weight of 15 kg for 1 week, and 7.5 kg for another 1 week in preoperative period. Then total laminectomy of C3 and C4 was performed. Interspinous ligament and ligamentum flavum between C4 and C5 were disrupted and band-like granulation tissue was adhered to dura at the level of C4-5 interspace. The cervical cord was intact.
Postoperatively traction was maintained with the weight of 7.5 kg for 2 weeks, and she was discharged in good condition.
